Form Category

Category of forms - Choose a category from the list based on the type of transaction you wish to perform.

Contract Number - Enter a contract number in the contract number field or choose a contract from the list.

Search Criteria

Sales Kit - The Sales Kit process uses a series of questions and answers to produce a packet of forms suitable for submitting new business, 1035 Exchanges/Transfers/Rollovers or Internal Product Exchange within Hartford.

Interview - The Interview process uses a series of questions and answers to produce a form or forms used to supplement or transact changes to existing annuity contracts.

Forms Library - The options provided will allow the user to select categories and sub-categories to narrow the results down to one form if so desired, but also provides the flexibility to not select a category or sub-category, in order to return a variety of forms.

Keyword - The Key Word Search process will allow the user to enter a word, phrase or number against which a search will be conducted to find all forms containing that word, phrase or number.

Non-Qualified Exchange Transfer - The replacement, via a 1035 Exchange, of a Non-Qualified life policy, endowment policy or annuity contract for another Non-Qualified annuity contract without the loss of tax deferred status. Or, the transfer of a Non-Qualified Mutual Fund, Certificate of Deposit (CD) or Brokerage Account to a Non-Qualified annuity contract.

Transfer/Rollover of Tax Qualified Plan - Transfer: A direct, tax-free, non-reportable movement of funds from one Qualified Plan (e.g. IRA, SIMPLE IRA, 403b etc.) to an annuity contract with the same Qualified Plan type. With a transfer, the participant does not take constructive receipt of the funds.

Rollover: A tax-free reportable movement of funds from one Qualifed Plan to another. With a rollover, the Qualified Plan type may change (e.g. 401k to IRA). The participant may take constructive receipt of the funds (indirect rollover) or have the funds paid directly to the new Qualified Plan (direct rollover).

New Sales Kit - Non-replacement business purchased using new money.

Contract State - State in which the client will sign the application.

Product - Select the Hartford Life Annuity product the client wishes to purchase.

New Plan Type - Select the desired Plan Type of the new Hartford contract being established.

Forms Library Search Results - Select forms in the "Available Forms" window then click the "Add" button to move forms to the "Chosen Forms" window. Multiple forms may be selecting by holding down the "CTRL" key.

Key Word Search

Key Word Search - Enter a form name, number or keyword in the Keyword field and click the "Search" button to search on a word or phrase.

Name Package

Name Package - Presents a view of all forms assembled in package based on answers provided in interview. The "Name of Package" field can be updated with a more meaningful name.

Open Forms

Open Forms - Forms maybe opened by clicking on the form title. Forms may also be emailed from this page by selecting the checkbox next to the forms or package of forms and then clicking on the "E-mail" button at the bottom of the page. To return to the beginning and gather more forms, click the "New Search" button at the bottom of the page
